These services are provided at 17 May Street, Hartford, Connecticut. Our Second Phase of Transitional Center services includes 4 hours a week of group and individual case management services in a 3.1 level of care. These services are provided in a residential setting that provides a safe and structured environment. The groups provided are more recovery and relapse-prevention-based as the men in the transitional program are now connected with outpatient-based community substance use and mental health treatment services.
Case management services are also provided to assist the men in the program with educational, vocational, and housing referral services so that they can re-enter their community or return to their family environment. Family therapy services are also available to ensure a healthy and beneficial return for the men to their families.
